Can we have additional fields available on the form to be added. <ol><li>I would like a checkbox field initially for a "I would like to be contacted about my feedback" confirmation checkbox for GDPR consent purposes to enable management to contact the customer about their feedback</li></ol><ol start="2"><li>HTML Embed - could we have a field that would accept a html embed so i can embed my own form created elsewhere?</li></ol>
