Private Feedback Form Additional Field Types

Can we have additional fields available on the form to be added.

1. I would like a checkbox field initially for a "I would like to be contacted about my feedback" confirmation checkbox for GDPR consent purposes to enable management to contact the customer about their feedback

2. HTML Embed - could we have a field that would accept a html embed so i can embed my own form created elsewhere?
