Add "LOCATION" to private feedback notifications

Clients are complaining that they have multiple locations, but that the location information isn’t contained in the private feedback notification that they’re getting. I think this would be a simple, useful addition to the emails.

Also, it would be nice to add a link directly to that section & location where the user can click to respond or review, + make the reply-to email from the notification the user’s email address, if there is one.
