add a description/note field to widget

That displays on the /displays-reviews widget cards for more information. I would use these for info about any specific customizations or which tags I am using and which page I am installing it on. It would be very helpful. Right now would have to use the name field to put all that info and it doesn’t display so nicely with a lot of text.
